To do this you will need

  1. The last four digits of your credit card
  2. Personal details on the account (name, address, date of birth)

You cannot use this form if:

  1. Your account is closed
  2. You want to INCREASE your credit limit and in the last 6 months you have either, requested a credit limit increase or you have opened your account
  3. We have no contact details on your account

You must be the Primary card holder to complete this form.
