Before you start

This form will let you...

• Request a change to your payment date for your Slyce card

To do this you will need

• The last four digits of your Slyce card
• Personal details on the account (name, address, date of birth)

Good to know

• We don't process payments every day. This means that sometimes we'll take payment just after your payment date, but never before.
• When your account has been open for 30 days you can ask to change your payment date. You can change it once every six months.
